| STEP 1: Download the App |
a) Open the App Store on your iPad (or iTunes) and search for naturalFORMS.
Make sure that you are signed in with your Apple ID. The App is FREE.
b) Touch the INSTALL button to install the application on your iPad.
c) naturalFORMS will install on the iPad, and the icon will be viewable on the main
screen of your iPad, as shown below.

| STEP 2: Gather required information |
a) On your iPad open Settings then About on the right your will see your Serial
Number. Write this number down you will need it for the registration form.
b) Close Settings
c) Open naturalFORMS™. In the middle of your screen you will see your EF#.
Write this number down you will need it for the registration form.

| Now that you have your iPad Serial Number and the naturalFORMSTM ID Number click on the link below it will take you to the Registration Page. |
